Abstract
Clinical education is an integral and essential part of nursing education, and 50% of the curriculum is dedicated to it. Curriculum design and clinical teaching methods for nursing educators are a continuous and necessary process that must be done using students' needs because because students' education is effective in providing safe and quality care to patients.
In psychiatric nursing, the examination and collection of data are important in formulating a nursing diagnosis and care plan for neuropsychiatric patients, and the psychiatric nurse is a key member of the treatment team, but due to the lack of a single form to examine the mental state in the form of a nursing process for training and evaluation nursing students in the mental health internship unit, the researchers developed a form for patient evaluation by the student along with a guide to complete it, so that in addition to education, student evaluation can also be done through it. This scientific letter aims to propose the integration of the patient evaluation form in all physical and mental dimensions for the training and evaluation of nursing students in the mental health nursing internship unit.
